PERTUSSIS BOOSTER SCHOOL LAW INFORMATION
An immunization law Assembly Bill 354 requires all students entering grades 7 to receive a pertussis booster vaccination prior to school entry. The required vaccine is called Tdap (tetanus, diphtheria, and acellular pertussis).
Required information from your healthcare provider is:
o student name
o date of birth
o vaccine type
o date of vaccination
o name of provider or clinic where the vaccination was received
Incoming 7th grade students must provide proof of vaccination prior to the first day of school.
To obtain pertussis vaccinations (Tdap), please contact your child’s usual provider. If you do not have a regular provider or health insurance, parents can call 2-1-1 or visit the website http://publichealth.lacounty.gov/ip/IZclinics/clinics.htm
for a list of low or no-cost vaccine providers. Families are asked to please phone clinics in advance to confirm hours and vaccine availability.
